Team Choice is seeking a bilingual, dedicated and experienced RN Care Coordinator to join our team. Reporting to the Director of Care, the RN Care Coordinator is responsible for planning, directing, and evaluating patient and staff, implementing standards of care, and development of field staff personnel. The Care Coordinator provides leadership and direction in all aspects of patient care. This individual will assist in the delivery of services including patient intake, assessments, delegations, scheduling, education, complaints, and ongoing monitoring.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Complete patient onboarding process in English or French as required.
  • Develop and maintain effective Care Plans to meet established goals.
  • Maintain shift report approvals, accurate records, progress notes, risk codes, schedules, and goals.
  • Ensure that the level of care established for patients is safe and appropriate and communicate with patient or substitute decision makers of any issues, concerns or care related matters as needed.
  • Respond to staff, client and family requests/concerns/incidents/complaints and complete incident investigations, action plans, disciplinary actions and or follow ups as required
  • Promote and maintain positive public relations and quality care by participating in joint meetings, care conferences, visits with patients..
  • Conduct patient satisfaction calls, update records and maintain logs.
  • Investigate complaints, issues and concerns, reports to management and follows up promptly following established procedures.
  • Perform other duties as required.

STAFF MANAGEMENT AND DEVELOPMENT 

  • Respond to field staff concerns, requests, and advocates on their behalf
  • Approve, train, and monitor special functions as outlined in delegation procedures and job descriptions.
  • Perform supervisory visits as needed for but not limited to training, delegation, safety, and performance issues. Ensure appropriate interventions and or follow-up actions are implemented and completed.
  • Oversee scheduling coordinators to ensure that patients are properly scheduled in a customer friendly and efficient way.
  • Supervise and evaluate Personal Support Worker performance and conduct disciplinary actions when required.
  • Identify and assist Personal Support Workers with support and compliance to meet Quality Assurance, Infection Control and Risk Management policies, procedures, education, and practices.
  • Provide ongoing support to staff with patient care web-based application support, orientation, coaching and mentoring.

 EDUCATION, EXPERIENCE & REQUIREMENTS

  • Registered Nurse with current unencumbered licensure to practice professional nursing in the province of Ontario.
  • Thorough knowledge of Personal Support Workers processes and practices.
  • Previous experience with Ontario Health @Home programs and processes
  • Excellent interpersonal skills and critical thinking skills.
  • Excellent customer service skills and ability to deal tactfully with patients, staff and community in English and French, both orally and written
  • Understands provincial and federal regulatory compliance.
  • French and English proficiency- Bilingualism essential
  • Ability to travel in Ottawa and surrounding area
  • Ability to be on call 1 week (7days) per month to provide phone support
  • Must be able to complete client home visits and have access to a reliable vehicle.
